ABSTRACT

Objective@#This study aims to develop an automated and objective tool to evaluate postural abnormalities in Parkinson’s disease (PD) patients. @*Methods@#We applied a deep learning-based pose-estimation algorithm to lateral photos of prospectively enrolled PD patients (n = 28). We automatically measured the anterior flexion angle (AFA) and dropped head angle (DHA), which were validated with conventional manual labeling methods. @*Results@#The automatically measured DHA and AFA were in excellent agreement with manual labeling methods (intraclass correlation coefficient > 0.95) with mean bias equal to or less than 3 degrees. @*Conclusion@#The deep learning-based pose-estimation algorithm objectively measured postural abnormalities in PD patients.

ABSTRACT

Acute vestibular neuritis (VN) is characterized by acute/subacute vertigo with spontaneous nystagmus and unilateral loss of semicircular canal function. Vestibular system in human is represented in the brain bilaterally with functional asymmetries of the right hemispheric dominance in the right handers. Spatial working memory entails the ability to keep spatial information active in working memory over a short period of time which is also known as the right hemispheric dominance. Three patients (patient 1, 32-year-old female; patient 2, 18-year-old male; patient 3, 63-year-old male) suffered from acute onset of severe vertigo, nausea and vomiting. Patients 1 and 2's examination revealed VN on the right side showing spontaneous left beating nystagmus and impaired vestibular ocular reflex on the right side in video head-impulse and caloric tests. Patient 3's finding was fit for VN on the left side. We also evaluated visuospatial memory function with the block design test in these 3 VN patients which discovered lower scores in patients 1 and 2 and the average level in patient 3 compare to those of healthy controls. Follow-up block design test after resolved symptoms showed within normal range in both patients. Our cases suggest that the patients with unilateral peripheral vestibulopathy may have an asymmetrical effect on the higher vestibular cognitive function. The right VN can be associated with transient visuospatial memory dysfunction. These findings add the evidence of significant right hemispheric dominance for vestibular and visuospatial structures in the right-handed subjects, and of predominant dysfunction in the hemisphere ipsilateral to the peripheral lesion side.

ABSTRACT

In Korea, current status of epilepsy and driving are challenging and there are lack of formal legal guidelines about driving in patients with epilepsy. According to the default standards in Korean Road Traffic law, patients with epilepsy are restricted or prohibited from driving except who are conditionally allowed to drive by the Aptitude Judgement Committee (AJC). Though the AJC consist of medical doctors and traffic officials, new regulation and guidelines are required for various type of seizure and characteristics of patients with epilepsy. This review outlines the current applicable legislation about epilepsy and driving in Korea as well as that of the overseas country calling for new laws to establish a consistent assessment.

ABSTRACT

Hypereosinophilic syndrome is a rare disorder involving the eosin concentration being elevated to more than 1500/µL in the peripheral blood for 6 months, and it causes various complications in the heart, skin, and nervous and respiratory systems. The simultaneous occurrence of neurological complications of hypereosinophilic syndrome is rare. Here we report a patient with hypereosinophilic syndrome who suffered from acute cerebral infarction and peripheral neuropathy during the same period.

ABSTRACT

OBJECTIVES: We report an outbreak of skin disorder in semiconductor packing workers. Through an evaluation of the degree of work-relatedness in this case, we aim to develop a method to overcome such health problems in the workplace. METHODS: A questionnaire investigating the job characteristics and skin symptoms was administered to 51 packing workers working in a semiconductor production factory and 49 questionnaires returned. The 16 subjects in the exposure group and 12 in the non-exposure group underwent doctor's examination and patch test. Two of 28 subjects were excluded in patch test. We investigated the work environment and carbon fiber level which was the potential causative agent. RESULTS: Forty-five (91.8%) of 49 packing workers complained of itching and stinging. Three of 14(21.4%) in the exposure group and 3 of 11 (27.3%) in the non-exposure group were positive at 48hr reading of patch test for carbon fiber and carbon fiber paper. None was positive at 96hr reading of patch test. Prominent carbon fibers were microscopically observed on the surface of the packing box and the fiber diameter, measured up to 6.0~7.5 micrometer, was likely to induce skin irritation to skin. To avoid worker's direct contact with to carbon fiber, carbon fiber paper box was substituted by a wax coated carbon fiber paper box was substituted for the original carbon fiber paper box, which caused and the workers' symptoms to of workers disappeared. CONCLUSION: The probably cause of this outbreak of skin disorder was mild irritant contact dermatitis for carbon fiber. To prevent outbreaks of such occupational skin disorder like this, workers should avoid direct contact with carbon fiber.

INTRODUCTION: In recent years, vascularized, i.e., living bone grafts, have been widely applied in the field of oral and maxillofacial surgery, as a method of treatment of congenital or acquired non-unions, and a large defects in mandible. The vascularized fibular graft has been especially used for this purpose because of its shape and mechanical strength. The postoperative hypertrophy of grafted fibula is of particular interest to us. MATERIAL AND METHODS: This study was undertaken to determine the volume change(indirect methods) and radiographic appearance of a free vascularized fibular graft as it responds to the mechanical and physiologic features of its new environment. In order to elucidate the long term effect on fibular mass after mandibular reconstruction, change in various method of volume change was utilized as indirect measure of change in long-term. RESULTS: The younger the patient, the more prominent and rapid the hypertrophy of the graft. the hypertrophy of the graft never exceeded the diameter of the recipient bone, except for callus enlargement after stress fracture of the grafted bone. CONCLUSION: Etiologic explanations for this phenomenon have not been clarified in the previeous literature. some of the factors implicated include a periosteal reaction or new bone formation, as seen at the onset of bone union after a fracture in a child, a reaction to the mechanical loading on the graft and a reaction to the circulatory changes resulting from the grafting procedure.

ABSTRACT

PURPOSE OF STUDY: Lingual nerve damage can be caused by surgery or trauma such as physical irriatation, radiation, chemotherapy, infection and viral infection. Once nerve damage occurred, patients sometimes complain taste change and loss of taste along with serious disturbance of tongue. The purpose of this study was to evaluate the effects of unilateral lingual nerve transection on taste as well as on the maintenance of taste buds. MATERIALS AND METHODS: Male Sprague-Dawley rats weighing 220-250g received unilateral transection of lingual nerve, subjected to the preference test for various taste solutions (0.1M NaCl, 0.1M sucrose, 0.01M QHCl, or 0.01M HCl) with two bottle test paradigm at 2, 4, 6, or 8 weeks after the operation. Tongue was fixed with 8% paraformaldehyde. After fixation, they were observed with scanning electron microscope(JSM-840A(R), JEOL, JAPAN) and counted the number of the dorsal surface of the fungiform papilla for changes of fungiform papilla. And, Fungiform papilla were obtained from coronal sections of the anterior tongue(cryosection). After cryosection, immunostaining with Galpha gust(I-20)(Santa Cruz Biotechnology, USA), PLCbeta2(Q-15)(Santa Cruz Biotechnology, USA), and T1R1(Alpha Diagnostic International, USA) were done. Immunofluorescence of labeled taste bud cells was examined by confocal microscopy(F92-300., Olympus, JAPAN). RESULTS: The preference score for salty and sweet tended to be higher in the operated rats with statistical significance, compared to the sham rats. Fungiform papilla counting were decreased after lingual nerve transaction. In 2 weeks, maximum differences occurred. Gustducin and T1R1 expressions of taste receptor in 2 and 4 weeks were decreased. PLCbeta2 were not expressed in both experimental and control group. CONCLUSION: This study demonstrated that the taste recognition for sweet and salty taste changed by week 2 and 4 after unilateral lingual nerve transection. However, regeneration related taste was occurred in the presence of preserving mesoneurial tissue and the time was 6 weeks. Our results demonstrated that unilateral lingual nerve damage caused morphological and numerical change of fungiform papilla. It should be noted in our study that lingual nerve transection resulted in not only morphological and numerical change but also functional change of fungiform papillae.

ABSTRACT

OBJECTIVES: To investigate the causative agents for the dermatological problems associated with the polypropylene manufacturing process. METHODS: The study subjects consisted of 44 male workers involved in the polypropylene manufacturing process. Twenty-seven of them had been exposed to additives, and the remaining 17 had not. Among the 27 exposed, 10 had been exposed previously, and 17 are currently being exposed through their duties. Patch test were performed with three different concentrations, 0.1%, 1% and 10% of 10 different additives that was diluted with Vaseline. RESULTS: 18 subjects (41%) had positive reactions to the patch tests. Exposed group had a higher reaction rate (51.9%) than the control group (23.5%). When the positive reactions were further classified, 7, 6 and 5 were compatible with probably irritant, probably allergic, and multiple hypersensitivity reactions respectively. The most frequent reactions were from Ultranox-626 followed by Millad 3988, Armoslip-E and Songstab Sc-100. CONCLUSIONS: When considering the significantly higher probability of an allergic reaction rate among the currently exposed group compared to non- or previously-exposed groups, the nature of dermatological problems in a polypropylene plant is probably allergic. Ultranox-626, Millad3988, Armoslip-E and Songstab Sc-100 were the main causatives agents found in the process.

ABSTRACT

OBJECTIVES: To describe the distributions of blood pressure level and risk group, and the current state of life style modification in the population of a large factory. METHODS: We surveyed 1,088 workers in a large factory from June 2002 to August 2002. Information related to hypertension was collected through a questionnaire and medical examination. We classified the blood pressure level and risk group using JNC-VI criteria, and the current state of life style modification and medication according to the blood pressure level in the entire study population. REULTS: The prevalence of hypertension was 30.1% in all workers. The distribution of blood pressure level was 656 (60.3%) in normal stage, 160 (14.7%) in high normal stage, 190 (17.5%) in stage 1, 67 (6.1%) in stage 2, and 15 (1.4%) in stage 3. The numbers of workers in risk groups A, B and C were 296 (27.2%), 690 (63.4%) and 102 (9.4%), respectively. Among all workers, 432 (39.7%) needed to modify their lifestyle; 106 of whom (24.5%) needed to go on hypertensive drug medication. Overweight subjects were more common in stage 2 than in other stages (p=0.02). The numbers of smokers and low-exercisers were signigicantly lower in the higher blood pressure group than the lower blood pressure group (p<0.01). Alcoholic drinking was evenly distributed in all blood pressure level. CONCLUSION: The prevalence of hypertension was 30.1% in all workers. The most common stage of blood pressure level was normal, followed by stage 1, high normal stage, stage 2, stage 3. The number of subjects in risk group B was more than that in the other risk groups. We recommend total worker education and the practice of lifestyle modification.
